Directions for filling out the order form
1. Fill out the order form completely.
2. Print out the order form after filling it out.
3. Sign under the payment portion of the order form if you wish to pay with a credit card.
4. Once you have completed, printed, and signed the form, either mail or fax it. If you have any questions, please call LadyRidersWear at 800-217-3526.
5. Please do not fax the order form unless you are paying with a credit card. If you wish to pay with a check or money order, please mail the form to us.
